JOB DESCRIPTION SUMMARY
The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and community.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Regularly re-evaluates patient nursing needs.
3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
4. Uses health assessment data to determine nursing diagnoses.
5. Develops a plan of care, which establishes goals based on nursing diagnoses and incorporates therapeutic, preventative, and rehabilitative nursing actions. Includes the patient and the family in the planning process.

1. Provides physical therapy services to patients according to a written physician’s plan of care. This may include, but not limited to:
A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in

1. Evaluates patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self care ability, work capacity, etc.) Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.
2. For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Absolute Home Health policy.
3. Develops treatment program and establishes goals for improved function. Communicates plan of care to
referring physician and other Absolute Home Health professionals.
4. May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with Absolute Home Health policy.

7. Maintain appropriate clinical records, clinical notes, and reports to physician any changes in patient’s

The speech-language pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement.

The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The Intake/ Referral Specialist is responsible for managing all aspects of the patient intake process.
The specialist is responsible for establishing and maintaining positive relationships with all community referral resources, patients and families. The specialist is responsible for responding to customer requests, and concerns as they pertain to the admission process and managing insurance verification and authorizations.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Performs all daily patient referral and intake operations including assisting with the direct oversight of the Agency Program and implementation of its intake policies procedures.
2. Remains familiar with all state, federal and special accreditation regulations pertaining to the referral intake process.
3. Assists in the evaluation and implementation of improved work methods and practices to ensure patients are admitted in accordance with the agency Policy and Procedures.
4. Establishes and maintains positive working relationships with current and potential referral sources.
5. Ensures the seamless transition of patients to home care by calling the patient or responsible party prior to discharge from Hospital to confirm and verify: Demographic information, Insurance information and Primary Care Physician or ordering home care physician, and Emergency contact
information.
6. Ensures reimbursement of home care services through direct oversight of insurance verification and authorization through. E solutions. (Medicare). Managed care; verify insurance eligibility with referral.
7. Verifies Primary Care Physician upon referral from the referral source (Care-Centrix, Molina, MD office).
8. Calls PCP to directly confirm that MD is PCP and will sign all home health orders for the particular patient.
9. Creates patient Chart in Kinnser. Responsible for creating patient charts. Providing as much information as possible. Requests HP, MD orders, current Medication list from Case Manager or
referral source from Care-Centrix, Molina, etc. (HP not available from referral source; contact
primary MD for most recent HP).
10. Upon referral, identifies potential staff member to act as Start of Care clinician; Calls clinician to confirm availability acceptance; then schedules SOC in Kinnser in patients chart with designated staff member.
11. Data entry: Responsible for data entry of all patient information.
12. Responsible for receiving daily labs; printing, submitting to Clinical Supervisor, Director or Clinical Coordinator for review; then fax to MD once lab is re-signed by Supervisor. After Sup sig and faxed to MD; upload to patient chart; write Kmail to clinician informing them of labs; if critical results, phone clinician and MD.
13. Participates in Case Conferences as needed.
14. Responsible for educating clinical staff as it pertains to the intake process, as needed.
15. Responsible for charting pertinent conversations as they pertain to the intake process in Kinnser
as needed.
16. Responsible for participating in coverage of after-hours, On Call/Triage phone program. Will accept phone calls, record in Triage log, M-F evenings and weekends. Intake Specialist will alternate with other office staff to ensure 24/7 patient coverage. Alternating weeks with other office staff members, and will Provide coverage over the weekend. Weekend coverage includes monitoring ECIN and responding to potential Referrals.
17. Assists the management staff with any other tasks that are necessary to help the operations of the agency. The incumbents may be requested to perform other tasks that are not directly related to intake/ referral but are deemed necessary at the time to maintain consistent operations of the agency.
