Certified Home Health Aide

JOB DESCRIPTION SUMMARY

The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed.
The home health aide will be assigned in a manner that promotes quality, continually and safety of a patient’s care.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Responsibilities of the home health aide include, but are not limited to, the following:

1. Providing personal care including:

A. Baths
B. Back rubs
C. Oral hygiene
D. Shampoos
E. Changing bed linen
F. Assisting patients with dressing and underdressing
G. Skin care to prevent breakdown
H. Assisting the patient with toileting activities
I. Keeping patients living area clean and orderly, as appropriate

2. Planning and preparing nutritious meals.

7. Assisting patient in the self-administration of medication.

8. Doing patients laundry, as appropriate.

9. Meeting safety needs patients and using equipment safely and properly (foot stools, side rails, etc.)

10. Reporting on patient’s condition and significant changes to the assigned nurse.

JOB DESCRIPTION SUMMARY

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

1. Provides direct patient care as defined in State's Name Nurse Practice Act.

POSITION QUALIFICATIONS

Registered Nurse

JOB DESCRIPTION SUMMARY

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and community.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

11. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).

2. Regularly re-evaluates patient nursing needs.

3. Initiates the plan of care and makes necessary revisions as patient status and needs change.

4. Uses health assessment data to determine nursing diagnoses.

5. Develops a plan of care, which establishes goals based on nursing diagnoses and incorporates

therapeutic, preventative, and rehabilitative nursing actions. Includes the patient and the family in the

planning process.

6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and

treatments as prescribed by the physician.

7. Counsels the patient and family in meeting nursing and related needs.

8. Provides health care instructions to the patient as appropriate per assessment and plan of care.

9. Identifies discharge planning needs as part of the care plan development and implements prior to

discharge of the patient

10. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate

Physical Therapist

JOB DESCRIPTION SUMMARY

The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Provides physical therapy services to patients according to a written physician's plan of care. This may include, but not limited to:

A. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in
development of the total plan of care. Evaluates home environment.

B. Assesses for muscle strength, mobility, gait, ROM - potential rehab.

C. Directing physical therapy treatment.

D. Instructing patients and families/caregivers in the use and care of therapeutic appliances.

E. Determine priority needs for physical therapy.

F. Reporting to physician patient's reaction to treatment or changes in condition.

2. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the therapy program for the patient by participating in case conferences.

3. May train patient in the use of prosthetic device.

4. Identifies patient and family/caregiver needs for other home health services and refers as necessary.

5. Prepares and submits clinical and progress summaries based on the attainment of goals.

6. Participates in discharge planning for patient.

7. Prepares and submits a clinical progress summary based on the attainment of goals as directed by the Organization policy.

8. Provides physical therapy consultation to home families/caregivers when indicated.

Occupational Therapist

JOB DESCRIPTION SUMMARY

The Occupational Therapist contracted or employed through Absolute Home Health is responsible for providing
occupational therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Evaluates patient’s functional status (muscle function, endurance, visual coordination, written and

verbal communication skills, self care ability, work capacity, etc.) Evaluates home environment for

hazards or barriers to more independent living. Identifies equipment needs. Participates in the

development of the total plan of care.

2. For patients who plan to return to work, the occupational therapist may perform work capacity

evaluation and may refer to specialized vocational training in accordance with Absolute Home Health

policy.

3. Develops treatment program and establishes goals for improved function. Communicates plan of care to

referring physician and other Absolute Home Health professionals.

4. May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness

or injury, in accordance with Absolute Home Health policy.

5. May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve

function.

6. May train patient in the use of prosthetic and/or orthotic devices.

7. Maintain appropriate clinical records, clinical notes, and reports to physician any changes in patient’s

condition. Submits these records in accordance with Absolute Home Health policy.

8. Maintains contact/communication with other personnel involved in the patient’s care to promote

coordinated, efficient care.

9. Attends and contributes to inservices, case conferences, and other meetings as required by Absolute

Home Health to ensure coordinated and comprehensive plans of care for the patients of Absolute Home

Health.

10. Identifies patient and family/caregiver needs for other home health services and refers as appropriate.

11. Instructs patient’s family/caregiver and other Absolute Home Health personnel in patient’s treatment regime

when indicated and appropriate.

12. Supervises Certified Occupational Therapy Assistants according to Absolute Home Health policy and state

regulations.

13. When therapy is the only skilled service, instructs, supervises and evaluates home health aide every (2) weeks.

POSITION QUALIFICATIONS

1. Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education

and Accreditation of the American Medical Association and the American Occupational Therapy Association.

2. Registered with the National Registration Examination of the American Occupational Therapy Association.

3. Two years of clinical experience as an occupational therapist. Community/home health experience preferred.

4. Current licensure in the State of California, CPR certification, auto insurance and valid driver’s license.

5. Management experience not required. Responsible for supervising Certified Occupational Therapy Assistants and

Home Health Aides.

6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing

skills per competency checklist.

7. Prolonged or considerable walking, standing. Able to lift position or transfer patients. Able to lift supplies and

equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform

required nursing skills.

Speech-Language Pathologist

JOB DESCRIPTION SUMMARY

The speech-language pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.

2. Identifies functional speech defects and establishes a plan of care to improve patient's function.

3. Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.

4. Recommends communication devices/aids as indicated.

5. Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.

6. As appropriate, participates in case conferences, inservices and other meetings required to ensure coordinated and comprehensive care.

7. Prepares and submits notes per organization policy.

8. Maintains clinical competency in speech language pathology practice and theory.

9. Identifies patient and family/caregiver needs for other services and refers as appropriate.

10. Instructs patient, family/caregiver and other Organization health care personnel in the patient's treatment regime as indicated.

11. May supervise home health aide in procedures directly related to the speech-language therapy plan of care.

12. Performs other duties as required to facilitate the delivery of speech language pathology services.

POSITION QUALIFICATIONS

1. Possesses a degree that includes an internship from an approved Master's program in Speech-Language Pathology, or as required by applicable law and/or regulation.

Referral/intake Specialist

JOB DESCRIPTION SUMMARY

The Intake/ Referra l Specia list is responsible for managing a ll aspects of the patient intake process. The specialist is responsible for establishing and maintaining Positive relationships with all community referral resources, Patients and families. The specialist is responsible for responding to customer requests, and concerns as they pertain to the Admission process and managing insurance verification and authorizations.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Performs all daily Patient referral and intake operations including assisting with the direct oversight of the Agency Program and implementation of its intake policies & procedures.

2. Remains familiar with all state, federal and special accreditation regulations pertaining to the referral I intake Process.

3. Assists in the evaluation and implementation of improved work methods and practices to ensure patients are admitted in accordance with the agency Policy and Procedures.

4. Establishes and maintains positive working relationships with current and potential referral sources.

5. Ensures the sea mless transition of patients to Home care by calling the patient or responsible party prior to Discharge from Hospita l to confirm and verify: Demographic information, Insurance information and Primary Ca re Physician or ordering Home Care physician, and Emergency contact information.

6. Ensures reimbursement of home care services through direct oversight of insurance verification and authorization through. E­ solutions. (Medicare). Managed care; Verify insurance eligibility with referral.

7. Verifies Primary Care Physicia n upon referral from the referral source (Ca re Centrix, Molina, MD office).

8. Calls PCP to directly confirm that MD is PCP and will sign all Home Health Orders for the particular Patient.

9. Creates Patient Chart in Kinnser. Responsible for creating patient chart. Providing as much information as possible. Requests H&P, MD orders, Current Medication list from Case Manager or referral source from Care-Centrix, Molina, etc. (H&P not available from referral source; contact primary MD for most recent H&P).

10. Upon Referral, Identifies potential staff member to act as Start of Care clinician; Calls clinician to confirm availability & acceptance; then schedules SOC in Kinnser in patients chart with designated staff member.

11. Data entry: Responsible for data entry of all patient information.

12. Responsible for receiving daily labs; Printing,submitting to Clinical Supervisor, Director or Clinical Coordinator for review; then fax to MD once labs a re signed by Supervisor. After Sup sig and faxed to MD; upload to Pt. chart; write Kmail to clinician informing them of labs; if Critical results, phone clinician and MD.

13. Participates in Case Conferences as needed.

14. Responsible for educating clinical staff as it pertains to the intake process, as needed.

15. Responsible for charting pertinent conversations as they pertain to the Intake process in Kinnser as needed.

16. Responsible for participating in coverage of after-hours, On­ Call/Triage phone Program. Will accept phone calls, record in Triage log, M-F evenings and weekends. Intake Specialist will Alternate with other office staff to ensure 24/7 patient coverage. Alternating weeks with other office staff members, and will Provide coverage over weekend. Weekend coverage includes monitoring ECIN and responding to potential Referrals.

17. Assists the Management staff with any other tasks that are necessary to help the operations of the agency. The incumbents may be requested to perform other tasks that are not directly related to Intake/ referral but are deemed necessary at the time to maintain consistent operations of the agency.

Scheduling

JOB DESCRIPTION SUMMARY

The Scheduling Specialist is responsible for managing all aspects of the patient scheduling process. The specialist is responsible for establishing and maintaining Positive relationships with all community referral resources, agency clinicians, Patients and families. The specialist is responsible for responding to customer requests, and concerns as they pertain to the scheduling process and managing insurance verification and authorizations.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Performs all daily Patient Scheduling operations including assisting with the direct oversight of the Agency Program as it pertains to the scheduling process and implementation of its scheduling policies & procedures.

2. Remains familiar with all state, federal and special accreditation regulations pertaining to the scheduling Process.

5. Ensures the seamless transition of patients to Home care by calling the patient and clinician after the Start of Care to confirm and verify the schedule outlined by the insurance company and or the SOC clinician.

  • b. Updates Patient Chart in Kinnser. Responsible for updating patient chart as needed. When receiving Pharmacy information or other information not obtained at SOC; will assist clinician in updating clinical records.
  • c. After SOC, Identifies potential staff member to continue as primary clinician; Calls clinician to confirm availability schedule & acceptance.
  • d. Data entry: Responsible for data entry of all patient Scheduling information.
  • e. May Assist with daily labs; Printing,submitting to Clinical Supervisor, Director or Clinical Coordinator for review; then fax to MD once labs are signed by Supervisor. After Sup sig and faxed to MD; upload to Pt. chart; write Kmail to clinician informing them of labs; if Critical results, phone clinician and MD.

6. Responsible for scheduling all needed tasks for clinicians including all Oasis Assessments (ROC, Recerts, Transfers, & Discharges), plus Transfers, Summaries, MD orders, etc.

7. Responsible for Monitoring Hospitalizations, entering data into Kinnser, Calling hospitals on daily basis for Pt. DC status, and clinician notification of Discharge.

8. Responsible for charting pertinent conversations pertaining to pt. care in Kinnser.

9. Responsible for participating in Case Conferences as needed.

  • a. Responsible for educating clinical staff as it relates to scheduling as needed.
  • b. Responsible for creating and Maintaining Human Resource files; obtaining needed staff information and uploading to Kinnser software; Staff notification of upcoming and delinquent requirements such as Licenses, certifications, Insurance, PPD, Health questionnaires etc.

Responsible for managing all aspects of organization marketing including managing the members of the marketing team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and
negotiating service contracts with managed care organizations, government agencies, and other payers.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Directs all daily marketing operations including providing direct oversight of the establishment and implementation of marketing initiatives.

2. Ensures maximum third party reimbursement through effective negotiation of service contracts with managed care organizations, government agencies, and other payers.

3. Assists the Executive Director/Administrator in establishing organization volume projections in the annual budget and in establishing allocations for the marketing department. Monitors allocation of resources according to budgetary limitations.

4. Continuously conducts market assessments and develops a comprehensive marketing plan designed to meet budgetary volume projections.

5. Employs marketing and promotional initiatives to achieve budgetary volume projections.

6. Establishes and maintains positive working relationships with current and potential referral and payer sources.

7. Negotiates service pricing with insurance Case Managers and other payers within established financial and credit parameters.

8. Recruits, selects, orients, and directly manages members of the marketing team.

10. Provides leadership in strategic planning including identifying opportunities for additional or improved services to address customer needs.

13. Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by Absolute Home Health Services.
